One improvement to Robert's answer to number 5: you can avoid the use of a script expression by using v20 of HTTP request and choosing "POST a body - Input job".

I could reproduce this and logged it as a bug. As a workaround you can two update rules in "Update XMP", the first one sets pdf:Keywords to an empty value and the second one sets the value.

Check the debug messages. There you will see the actual command that is executed. That will usually give you an indication of what went wrong. On Mac you can use "Execute command" at the beginning of the flow to "touch" the file. That updates the timestamp of the file. On Windows you can apparently do it with a variant of the copy command: copy /b filename.ext +,,

The reason is obvious: the PDF was created by an application/producer that does not always create a correct PDF.
What is wrong with the file I can only tell after having seen it. Can it be fixed? The fact that something is missing does not sound promising, but you never know.
